2022 Confirmation registration

Use this form to register candidates for 2022 Confirmations being held in April and May. The deadline to register both youth and adults is 5 PM on Thursday, March 31, 2022.
Youth must be in 8th grade or higher to be eligible for Confirmation.

COVID-19 Note - These services may be relocated or rescheduled, if COVID risk levels increase this Spring. To keep risk lower at these gatherings, the number of candidates is limited to 25 for each service. Each candidate is allowed to bring 4 guests (in addition to their clergyperson).

What constitutes a complete registration? In addition to the required fields for Church Information, the following information is required for each candidate:

  • Full name as it should appear on the Confirmation Certificate, last name first
  • Denomination at Baptism
  • Date of birth
  • School grade (for youth), or indicate adult
  • If youth, parent/guardian name(s)
  • Contact email, phone and address

The name of the clergy person (or, if no clergy, the warden) who should be listed on, and will sign, the Official Acts Form for the Bishop's Office.

You may register up to 15 candidates below. To register more than 15, please submit this form again for the additional candidates.
If attending the diocesan Confirmation Retreat is part of your curriculum and requirements for Confirmation, by selecting Yes, you are notifying Episcopal House staff to use the candidate information entered here to initiate the Confirmation Retreat registration process. This will enable a smoother, more expeditious registration process for both retreat participants and staff.
